Billing and Insurance Policy
We accept most of Minnesota’s major health insurance plans. Please, check with you individual health plan for the most current information about our status as a provider with that plan.

You will need to present your insurance card at each appointment. Failure to have insurance information may require payment at the time of service. If you insurance has changed since your last appointment, please be sure to let us know.
Referrals: Many insurance plans may not pay for procedures or visits that are recommended or ordered by your health care provider. On occasion, a provider may recommend treatment by another provider outside of our clinic. If your health insurance plan requires a referral, please be aware that, since our clinic is not considered a primary care clinic, but a specialty clinic, we are unable to provide a medical referral. Only your primary care clinic can provide medical referrals. If referrals are required, always secure your referral prior to scheduling specialty procedures. This will prevent any unexpected delays in payment or denial of services.
Coding for Services: All health care providers bill for services using billing codes. These codes tell the insurance company what was done and why. Routine and preventive services cause confusion for many patients. It is not uncommon for patients in the course of a visit to receive both treatment for problems and preventive service. When this occurs, we are required to charge for both services.
Please be sure that you are familiar with what your insurance plan does and does not cover and when referrals are required.
Co-Pays: Co-pays are due at the time of services. It is your responsibility to know which service requires co-pays. If you are unsure of your co-pay requirements, please contact your health plan.
Self-Pay: Patients with no insurance coverage are required to make payment at check-out. Please call our billing office at 651-209-8109 for our special rates.
Credit Policy: Payment is due upon receipt of your first billing. If you cannot pay the amount in full, a minimum monthly payment of 20% of the outstanding balance is required. If for some reason you cannot meet these requirements, please contact our business office for payment arrangements.
No Show Policy: All cancellations need to be made 24 hours in advance. We reserve the right to charge you a missed appointment fee of $30. This fee will need to be paid prior to making any other appointments with Comprehensive Healthcare for Women, P.A. Three (3) missed or reschedule appointments are grounds for patient discharge.
