Patient Registration
Please arrive a few minutes before your scheduled appointment to allow the receptionists to assemble your chart and make necessary copies of your information. If you had any testing done at your primary physicians' office please bring a copy of the results along to your appointment. Please be sure to bring your insurance card to every visit. Office visit co-payments required by your health insurance plan are due at the time of service.
Create Your Online Account
It's easy to create your secure online account in the Patient Portal. Once you do, you'll be able to complete all forms electronically before you come in for your next office visit. Plus you'll be able to pay your bill online and receive appointment reminders via email. And we'll begin offering online appointment scheduling in the near future.

We accept most of Minnesota’s major health insurance plans. Please, check with your individual health plan for the most current information about our status as a provider with that plan.
